Please list at least two points of contact in the event of emergency situations which might alter the conditions of the film permit. At least one contact person must be located within 20 miles of the city.

This agreement is made and entered into as of this _____________ day of ___________________, 20____, by and between the City of Kenner (hereinafter referred to as CITY) and _________________________________________________ (hereinafter referred to as APPLICANT) in connection with the application to City for the issuance of a filming permit.
As a condition to the issuance of such filming permit by City, Applicant agrees to defend, indemnify, and hold the City harmless from and against any and all losses, liability, claims, action, causes of actions, suits, judgments, settlements, costs, or expenses (including but not limited to attorney fees as approved by a court, and responsible investigative, discovery and court costs), involving injuries to or death of any person, including injury to Applicant's employees or damage to property, including but not limited to any property of permittee or City occurring on or about the filming area, and all claims which arise from or are connected with the negligent performance of or failure to perform work or other obligations of this agreement, or are caused by the negligent acts of the City, its Councilmembers, officers, agents, or employees, except such injury, death, or damage as is caused by or arises out of or is in any way connected with the following: City's sole negligence or willful misconduct, but not from City's mere comparative or contributory negligence; and all expenses of investigating and defending against same.

For your reference, an application has been made with the City of Kenner for filming within the vicinity of your property. It is the production company's responsibility to ensure that there is a minimum of disruption to residents, occupants, businesses, and City employees where filming occurs. This includes ensuring residents, owners, and customers' access to the respective premises and pedestrian and vehicular access to adjoining properties.
